U.S. Customers: CBP Form 5106 and Duties
For now, these changes appear to impact primarily high-value orders over 800 USD, but we expect they may apply to all shipments in the coming days. As a result, FedEx has begun asking us to have some customers complete a Create/Update Importer Identity Form, also known as CBP Form 5106.
If FedEx contacts us regarding your shipment, we may reach out and ask you to fill in this form. You can download it [here] or directly from the U.S. Customs and Border Protection website. You do not need to fill in this form unless we or FedEx ask you to do so.
The form may look intimidating, but if you’re an individual (not a business), you’ll only need to complete a few specific sections as listed below. Once filled, please send the form to paperwork@fedex.com and include your airway bill (AWB) number in the subject line. This AWB number will be provided by us once your order ships, and FedEx should also email it to you directly.
Please note that the form has to be returned to FedEx as soon as possible as they state they would sent the package back to Japan 3 days after.
What a Regular (Non-Business) Customer Should Fill In on Pages 1 and 4 of the CBP Form 5106
- 1A. Full legal name (e.g., Jane Doe)
- 1B. Social Security Number (or write “NONE” if not available)
- Leave 1C and 1D blank (these are for business aliases)
- 1E. Check the box: “I wish to be assigned a CBP Number” only if you don’t want to or can’t use your SSN
- 1G. Check “Individual”
- 1H. Select “1–4 per year” (typical for personal imports)
- 1I. Check “Consignee/Ultimate Consignee” (you’re the final recipient of the package)
- 2A. Fill in your mailing address: Street, City, State, ZIP, and Country ISO code (“US”)
- Check the type of address (usually “Residence”)
- 2B. Leave blank if it’s the same as your mailing address
- 2C. Phone number
- 2E. Email address
- Printed name and title (e.g., “Jane Doe” and “Self” — or leave “Title” blank)
- Signature
- Telephone number
- Date
